Transcripts Requests

TRANSCRIPTS (Requests are for COA Independent Study Students Only - Not for Virtual Academy "Thematic" Students)
 
PLEASE NOTE: This request portal is ONLY for City of Angels Independent Study students and NOT for Virtual Academy students. Virtual Academy students can request transcripts at [email protected] or 213 241-3180. 
 
To get a record of your grades at City of Angels Independent Study School, you should:
  1. Click on the link located at the bottom of this page and print out the form you will need.
  2. Fill out all of the areas in "Student Information" and "Type of Transcript". 
  3. Under "Distribution", indicate how you wish to receive your transcripts, either by picking them up personally at our Admin Hub or by mail or fax.
  4. Be sure to fill out the correct address if receiving by mail and the correct fax number with area code if you are requesting that your transcripts be faxed.
  5. Sign and date the form.
  6. Mail the completed form and copy of a valid photo id to:

     7. You may also fax your request to (323) 261-1001.  Be sure to include a copy of a valid photo id.
 
PLEASE NOTE: If you were enrolled at City of Angels BEFORE 2014, you must contact LAUSD Student Records and Data Management (SRDM) and order transcripts online.
