To get a record of your grades at City of Angels School, you should:
- Click on the link located at the bottom of this page and print out the form you will need.
- Fill out all of the areas in "Student Information" and "Type of Transcript".
- Under "Distribution", indicate how you wish to receive your transcripts, either by picking them up personally at our Admin Hub or by mail or fax.
- Be sure to fill out the correct address if receiving by mail and the correct fax number with area code if you are requesting that your transcripts be faxed.
- Sign and date the form.
- Mail the completed form and copy of a valid photo id to:
